WA HCA 13-847: Home Health Authorization Request

Washington's Health Care Authority uses the HCA 13-847 for Apple Health fee-for-service home health authorization, mainly limitation extensions for skilled nursing and therapy visits and requests for clients with AEM coverage. A clinician meets it because the plan of care, including provider orders, must be attached. HCA reviews the client-specific medical justification and why the client cannot get skilled care in the community.

Who completes it

The home health agency completes the form, including agency and clinical contacts, units used and requested, related diagnoses and the justification. The ordering practitioner's plan of care and orders go with it.

Submitting and payment

How to submit it
Submit online through ProviderOne direct data entry, or fax it with the HCA 13-835 General Information for Authorization form as page one and no separate fax coversheet.

Good to know

  • PT, OT and social work units are 15 minutes each, while one speech therapy unit equals one visit regardless of length.
  • Clients with AEM coverage need prior authorization, and only two visits are allowed if authorized; explain how the service relates to the emergency condition.
  • The form asks for the estimated time the client will receive home health services.
  • HCA lists the Home Health Agency Attestation, HCA 13-0114, as a related form for home health agencies submitting these requests.
