WA HCA 13-835A: Pharmacy Information Authorization

Washington's Health Care Authority uses the HCA 13-835A for fee-for-service Apple Health pharmacy authorization requests, and for reimbursement rate adjustments on prescriptions not signed dispense as written. The form is faxed to HCA's pharmacy authorization unit, which sends a confirmation fax when it can identify the sender.

Who completes it

The prescriber or the pharmacy completes and types all fields, including pharmacy and prescriber NPIs, drug, NDC, quantity, days supply, directions, diagnosis and a comments section on medical necessity.

Submitting and payment

How to submit it
Fax the typed form and any supporting documents to HCA's pharmacy authorization fax line printed on the form.
Who pays
Org code 522 requests a pharmacy rate adjustment and needs the actual acquisition cost and average wholesale price per unit plus the wholesaler.

Good to know

  • Only one NDC can be requested per form, entered in 11-digit format with leading zeros; requests by UPC code are not accepted.
  • For changes or extensions, choose authorization type 2 (update) and enter the previous authorization number.
  • Patient residence code 11 flags a hospice client whose requested drug is not related to the terminal illness.
  • Apple Health uses one preferred drug list, but managed care members' prior authorization requests go to their plan, not HCA.
