WA HCA 13-772: Hearing Aid Authorization Request

Washington's Health Care Authority requires the HCA 13-772 when a provider requests Apple Health fee-for-service authorization for hearing aids, either as a limitation extension or an exception to rule. A clinician meets it when a client's hearing aid request falls outside standard program limits. HCA reviews the current audiogram, the explanation of why authorization is needed and answers about the client's functional needs.

Who completes it

The audiologist or hearing aid provider completes the form, including clinic and billing provider details, ProviderOne client ID, ICD-10 diagnoses, procedure codes and the clinical justification.

Submitting and payment

How to submit it
Fax the form with a typed HCA 13-835 General Information for Authorization form as the cover sheet, or enter the request in ProviderOne by direct data entry.

Good to know

  • Record the audiogram date and thresholds at 1000, 2000, 3000 and 4000 Hz for each ear, with the four-frequency average, and attach a copy of the audiogram.
  • One question asks whether the client already received one or more hearing aids from Medicaid in the past five years.
  • Other questions cover vision loss or legal blindness, other disabilities, tinnitus, prior hearing aid success, school, work, safety in the community and work-related injury.
  • Explain functional limitations and extenuating circumstances; page 3 gives extra space for answers.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does the Washington hearing aid request need a cover sheet?

Yes. When faxing, the HCA 13-772 must go with a typed and completed General Information for Authorization form, HCA 13-835, as its cover sheet. The 13-835 carries the program and service type codes, such as Org 508 for medical with service type HEA for hearing aids, along with the client and provider details.

What is the difference between a limitation extension and an exception to rule?

The form asks you to check one and explain why the request is being submitted, for example that the service is not covered, so an exception to rule is requested, that a limitation extension is needed, or that the client does not meet program criteria. The clinical justification then supports that request.
