Washington HCA 13-747 Hospital Bed Evaluation

Washington Apple Health uses this two-page evaluation when a hospital bed needs authorization. The provider names the bed type requested, from fixed height to extra heavy duty, the diagnoses with ICD-10 codes, hours per day in bed, and why a simpler bed or positioning aids will not meet the client's needs. It is submitted with the HCA prescription form 13-794.

Who completes it

The prescribing practitioner or evaluating therapist completes the clinical questions and signs with name and credentials. Supplier name, phone and billing NPI are also entered in Section 1.

Submitting and payment

How to submit it
Submit online through the ProviderOne Portal with the HCA prescription form 13-794 and supporting documentation, or fax it to the Authorization Services Office behind a General Information for Authorization form 13-835 cover page and the 13-794.

Good to know

  • Caregiver convenience is not considered medically necessary, so the reason a fixed or adjustable height bed will not work must rest on the client's condition.
  • For heavy duty and extra heavy duty beds, enter the client's weight and the date it was measured.
  • The form asks whether pillows, bolsters or foam wedges were tried and the outcome, whether emergent position changes are needed, and whether the client is at aspiration risk.
  • HCA has separate forms for related equipment, including low air-loss therapy systems (13-728) and negative pressure wound therapy (13-726). Current revision: 04/24.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What forms go with the Washington hospital bed evaluation?

The HCA prescription form 13-794 and any supporting documentation. For a fax submission, the General Information for Authorization form 13-835 goes first, then the 13-794, then the completed HCA 13-747 and supporting records. Online submissions go through the ProviderOne Portal.

Does Apple Health cover a semi-electric hospital bed for caregiver convenience?

The form states that caregiver convenience is not considered medically necessary. The provider has to explain why a fixed or adjustable height bed would not meet the client's needs, and the form asks about immediate position changes related to the medical condition and the risk of aspiration.
