Trustmark A112-2505 Pregnancy Disability Physician Form

Trustmark Voluntary Benefits uses this 1-page statement for pregnancy claims under worksite disability coverage. The obstetric provider records the last menstrual period, first treatment date, complications, the date the patient was advised to stop working, delivery date and type, and any period of reduced capacity. Trustmark pairs it with the claimant's own pregnancy claim form.

Who completes it

The physician completes and signs the whole Attending Physician Statement, including specialty and contact details. The patient completes the separate pregnancy claim form, A112-2505, posted on the same Trustmark page.

Submitting and payment

How to submit it
Fax or email it to Trustmark Voluntary Benefits using the claims submission contacts printed at the top of the form, with copies of medical records for the claim condition, including treatment notes and test results.
Who pays
The form lists no fee, billing code or Trustmark payment for completing it.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
