Trustmark A112-2515 Early Life Benefit Physician Form

Trustmark uses this 1-page Attending Physician's Statement when an insured with a terminal illness asks for an early (accelerated) life insurance benefit. The physician records the diagnosis and ICD code, onset and first treatment dates, treatments, medications and any hospitalization. The key line certifies that, after medical review, the patient is terminally ill and expected to survive only a stated number of months.

Who completes it

The attending physician completes and signs the whole statement. The insured files the separate Early Life Insurance Benefit (Terminal Illness) claim form, A112-2515, posted on the same Trustmark page.

Submitting and payment

How to submit it
Send it to Trustmark's life claims unit by fax, email or mail, using the contacts printed at the top of the form.
Who pays
The statement lists no fee, billing code or Trustmark payment for completing it.

Good to know

  • The prognosis sentence states that after a thorough, extensive medical review you have concluded the named patient is terminally ill, with a blank for months of expected survival.
  • The form also asks whether the patient smoked or used tobacco in the past 36 months and for the name and address of any referring physician.
  • New York residents use a different Trustmark form, the Accelerated Death Benefit Claim Form P321-9, listed in the New York section of the forms page.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
