Trustmark A112-2512 Waiver of Premium Physician Form

Trustmark uses this 3-page Attending Physician Statement when an insured applies to have life insurance premiums waived because of disability. The physician documents history, diagnosis with subjective and objective findings, treatment dates and medications. The form then grades physical and mental or nervous impairment on 5-class scales and asks whether the patient is totally disabled from their own job and from any other work.

Who completes it

The attending physician completes all nine items and signs page 3. The insured files the separate Begin Waiver of Premium claim form, A112-2512, posted with this statement on Trustmark's Voluntary Benefits forms page.

Submitting and payment

How to submit it
Send the statement to Trustmark's life claims unit by fax, email or mail, using the contacts printed on each page, and attach copies of medical records for the claimed condition, including treatment notes and test results.
Who pays
The statement lists no fee, billing code or Trustmark payment for completing it.

Good to know

  • Physical impairment runs from Class 1, no limitation (0 to 10%), to Class 5, severe limitation; Class 3 means capable of clerical or sedentary work (35 to 55%).
  • The form asks whether treatment will substantially improve functionality and employability, and whether the patient smoked or used tobacco in the past 36 months.
  • Prognosis is asked for the patient's own job and for any other work, with recovery estimates of 1 month, 1 to 3 months, 3 to 6 months or never.
  • If your practice needs its own authorization to release records, give it directly to the patient. Continuing waivers use A112-2806; New York uses P321-11.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What impairment classes does Trustmark use for waiver of premium?

Physical impairment is graded Class 1 (no limitation) through Class 5 (severe limitation), with Class 2 light manual activity and Class 3 sedentary work. A separate mental or nervous scale runs from Class 1, able to function under stress, to Class 5, significant loss of psychological and social adjustment.

Does the Trustmark waiver form ask whether the patient can manage money?

Yes. It asks whether you believe the patient is competent to endorse checks and direct the use of the proceeds. It also asks whether you, the physician, are related to the patient and, if so, what the relationship is.
