Trustmark A112-2504 Disability Claim Physician Form

Trustmark Voluntary Benefits needs this 2-page statement to pay a disability income claim under coverage bought at the patient's workplace. The physician documents onset and treatment dates, primary and contributing diagnoses with ICD-10 codes, objective impairment, and restrictions and limitations. Page 2 is where the physician certifies total or partial inability to work, with dates and a return to work prognosis.

Who completes it

The attending physician completes both pages and signs page 2. The claimant and employer complete the separate initial claim form, A112-2504, which Trustmark posts alongside this statement.

Submitting and payment

How to submit it
Fax or email the statement to Trustmark Voluntary Benefits using the claims contacts printed at the top of each page, and attach copies of all medical records for the claim condition, including treatment notes and test results.
Who pays
The statement lists no fee, billing code or Trustmark payment for completing it.

Good to know

  • Restrictions are activities the patient should not perform and limitations are activities the patient cannot perform; both must be based on the documented impairment.
  • The form asks whether the patient can do some work but no more than 50% of their regularly scheduled job, and for what period.
  • Section III lets you certify total or partial inability to work with dates, or refrain from certifying work capacity at this time.
  • Pregnancy claims add estimated and actual delivery dates and delivery type. Continuing claims use the A112-2516 continuance form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can I decline to certify disability on the Trustmark A112-2504 statement?

Yes. Section III offers a third option: 'I refrain from making a certification regarding work capacity at this time.' The other two options certify total or partial inability to work, each with from and to dates and a prognosis for return to work.

What does Trustmark mean by objective evidence of impairment?

Section I asks you to describe objective evidence for loss of physiologic, mental or anatomic function, including pertinent physical exam and diagnostic results. Section II then asks for the restrictions and limitations based on that documented impairment.

Who completes the rest of the Trustmark disability claim?

The claimant and employer use the initial claim form A112-2504, posted on Trustmark's Voluntary Benefits forms page next to this separate physician statement. The statement itself also asks whether another physician referred the patient and whether the condition relates to the patient's employment.
