Trustmark A112-2516 Disability Continuance Claim Form

Trustmark Voluntary Benefits uses this 10-page packet to keep paying an open worksite disability claim. The policy owner reports current impairment, daily activities, hospital stays, other treaters and other benefits. The attending physician's 2-page statement (A112-2516-APS) updates diagnosis, treatment, status and restrictions, then certifies total or partial inability to work with dates and a return to work prognosis.

Who completes it

The physician completes the Attending Physician Statement on the last two pages and signs page 2 of the statement. The policy owner completes Sections A and B, the disclosure authorization and the claim submission signature.

Submitting and payment

How to submit it
Fax or email it to Trustmark Voluntary Benefits using the claims contacts printed on the form, with copies of medical records for the claim condition, including treatment notes and test results.
Who pays
The form lists no fee, billing code or Trustmark payment for the physician's statement.

Good to know

  • The status line asks whether the patient has recovered, improved, not changed or retrogressed, alongside first and last visit dates and visit frequency.
  • Restrictions (activities the patient should not perform) and limitations (activities the patient cannot perform) must rest on the documented impairment described in Section I.
  • The statement asks whether the patient is competent to endorse checks and direct the use of the proceeds.
  • Trustmark labels this form the continuing claim; the first claim uses A112-2504 and its separate physician statement.
