Sun Life GSTDFM-5145: STD Provider Update Questionnaire

Once a psychiatric short-term disability claim is open, Sun Life uses GSTDFM-5145 to get periodic updates from the treating provider. The three-page questionnaire asks for mental status exam results from the last visit, progress with treatment, current DSM diagnosis, symptoms preventing the claimant's job duties, impairment level, medication changes, psychotherapy status and the target part-time and full-time return dates.

Who completes it

The treating behavioral health provider completes the whole questionnaire and signs with name and specialty. Section 1 identifies the claimant, employer and group policy number.

Submitting and payment

How to submit it
The completed update goes to Sun Life Group STD Claims by email, mail or fax, with the patient's name and claim number.
Who pays
The questionnaire sets no fee and does not say who pays for completing it.

Good to know

  • Progress with treatment should be specific, with examples; the form asks this directly alongside the mental status findings from the last office visit.
  • If the claimant takes no psychiatric medication or is not in psychotherapy, the form asks the provider to state the reason.
  • The provider names the prescriber and therapist with phone numbers so Sun Life can confirm treatment, and gives the last and next appointment dates.
  • Edition 09/25. It follows the initial Short Term Disability Provider Questionnaire, GSTDFM-5144, on the same claim.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What return-to-work questions are on Sun Life GSTDFM-5145?

Section 4 asks whether the provider and claimant have discussed return to work, whether it is one of the treatment goals and why not if it is not, what specific actions the provider is taking to support it, and the target return date for part-time and full-time work.

What impairment scale does GSTDFM-5145 use?

The provider rates the claimant's current degree of impairment as none, slight, moderate, marked or severe. That rating sits beside the description of specific symptoms that keep the claimant from performing the material and substantial duties of their occupation, with examples where possible.
