Sun Life GLTDFM-11245: LTD Medical Update

During an open Sun Life long-term disability claim, the insurer periodically asks the attending physician for a four-page medical update. GLTDFM-11245 covers the last and next examinations, diagnosis and reported symptoms, whether the patient has full-time or part-time work capacity, when a full-time return is expected, how often the patient can stand, walk, sit, lift and carry, and other restrictions with their expected duration.

Who completes it

The attending physician completes Sections 2 and 3 and signs with tax ID and specialty. The claimant's name, birth date, policy and claim numbers fill Section 1; there is no separate patient section.

Submitting and payment

How to submit it
The physician returns the update to Sun Life by fax, email or mail, including the patient's name and claim number. Full records for the period Sun Life names may be sent instead of the form.
Who pays
The form states that the patient is responsible for any cost to complete it.
