Sun Life GDIFM-11252: Behavioral Health Update

Sun Life sends GDIFM-11252 to the behavioral health provider when it needs a current picture of a disability claim based on a psychiatric or cognitive condition. The provider reports the last and next visits, current ICD-10 diagnoses, symptoms and exam findings, whether the patient has full-time or part-time work capacity, actionable treatment steps taken, coordination of care and specific restrictions and limitations.

Who completes it

The treating behavioral health provider completes Sections 2 and 3 and signs with tax ID and specialty. The claimant's name, policy and claim numbers go in Section 1; the patient does not complete a separate part on this form.

Submitting and payment

How to submit it
The provider returns the form by mail, fax or email to Sun Life as listed on page 4, with the patient's name and claim number. If the form cannot be completed, Sun Life accepts full records for the stated period instead.
Who pays
The form states that the patient is responsible for any cost to complete it.

Good to know

  • Instead of completing the form, the provider may send all medical records, exam notes, findings, test results and the current treatment plan from the date Sun Life writes in to the present.
  • Sun Life asks what actionable treatment steps have been taken, giving examples such as increased appointments, medication changes or a higher level of care.
  • The provider separates restrictions, activities the patient should not do, from limitations, activities the patient cannot do, and gives the expected duration of each.
  • The provider marks whether the opinion rests on the patient's self-reported severity of symptoms or on objective findings. Edition 12/25.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can I send records instead of completing Sun Life's GDIFM-11252?

What work capacity questions are on GDIFM-11252?

The provider answers whether the patient has full-time work capacity, defined as 8 hours a day, 5 days a week, and whether there is part-time transitional capacity with hours and days per week. The form also asks when a full-time return is expected and whether vocational support would help.
