Sun Life GSTDFM-5144: STD Provider Questionnaire

When a short-term disability claim rests on a psychiatric condition, Sun Life sends the treating provider GSTDFM-5144 to understand the condition, treatment and return-to-work goal. The provider explains how long the condition has existed, what changed so the claimant can no longer work, stressors in and outside work, mental status findings, DSM diagnosis, impairment level, medications, psychotherapy and the target return date.

Who completes it

The treating behavioral health provider completes the whole questionnaire and signs with specialty. The claimant's identifying details and group policy number go in Section 1; the claimant answers Sun Life's separate claimant questionnaire, GSTDFM-7202.

Submitting and payment

How to submit it
The completed questionnaire goes to Sun Life Group STD Claims by email, mail or fax, with the patient's name and claim number.
Who pays
The questionnaire sets no fee and does not say who pays for completing it.

Good to know

  • Question 2b asks what changed so that the claimant could work with the condition before but cannot now, which is often the central question in the review.
  • The provider describes the specific symptoms that prevent the material and substantial duties of the claimant's occupation, with examples, and rates impairment from none to severe.
  • Sun Life notes that return to work is customarily part of an ongoing treatment plan, and asks what specific actions the provider is taking to support it.
  • Edition 09/25. Use GSTDFM-5145, the update questionnaire, for later check-ins on the same claim.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What treatment details does Sun Life's GSTDFM-5144 ask for?

Section 3 asks how long the claimant has been in treatment, current psychiatric medications with dosages or why none are taken, recent medication changes, the prescriber's name and phone number, whether psychotherapy is ongoing and its type, the therapist's details, and the last and next appointment dates.

How is GSTDFM-5144 different from GSTDFM-5145?

GSTDFM-5144 is the initial questionnaire and includes condition history: how long it has lasted, what changed, and work and non-work stressors. GSTDFM-5145 is the update version, which drops those history questions and focuses on current mental status, progress, medications, therapy and the return-to-work goal.
