Sun Life GVCDFM-3044: Customized Disability APS

Sun Life's Customized Disability product is a voluntary worksite income plan, and GVCDFM-3044 is the physician's part of its claim. The nine-page statement has separate paths: a pregnancy section, a physical conditions section covering diagnosis, objective findings, treatment, progress, restrictions and prognosis, and a behavioral health section covering DSM diagnosis, impairment class, treatment history, testing and response to treatment.

Who completes it

The attending physician completes, signs and dates the statement, choosing the physical or behavioral health path. The employee and employer complete separate statements, GVCDFM-3043 and GVCDFM-3042.

Submitting and payment

How to submit it
The physician must submit the statement directly to Sun Life, by mail, fax or email as listed on the form, with the patient's name and claim number.
Who pays
The form states that the patient is responsible for any costs associated with its completion.

Good to know

  • For a normal pregnancy, only the pregnancy section is needed. Complications such as spotting or placenta previa call for ultrasounds and prenatal records with the statement.
  • Section 7 notes that a copy of the employee's formal job description is required for all claimants.
  • The physical impairment scale uses lifting definitions for medium, light and sedentary capacity; the form also asks whether the patient is released to their own occupation or any occupation.
  • A stamp or a signature from anyone other than the examining physician is not acceptable, and the physician must give a full address and tax ID. Edition 11/22.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does Sun Life's GVCDFM-3044 have a behavioral health section?

Yes. Physical-condition claims skip it, while behavioral health claims complete it instead of the physical sections. It asks for a current DSM diagnosis, mental impairment class 1 to 5, onset and treatment history, hospital or program stays, chemical dependency, psychological testing, medications and whether the patient can endorse checks.

What prognosis options does GVCDFM-3044 offer?

Both the physical and behavioral health sections ask how long the limitations will apply, with check boxes for an estimated 6 weeks, 8 weeks, 12 weeks or longer. The physical section also asks when the patient should be able to return to work full-time or part-time.
