Sun Life GDIFM-11136: Disabled Dependent Certification

Sun Life group plans can keep covering an unmarried dependent child past the usual age limit when the child cannot hold self-sustaining employment because of a mental or physical disability. GDIFM-11136 is how the parent applies: the employee completes the first page and the dependent's physician certifies the disability, its nature, the age it began and the prognosis on the second.

Who completes it

The employee completes Sections 1 to 3 with policy, dependent and marital details and signs. The dependent's physician completes Section 4, Physician assessment of disabled Dependent, and signs the Section 5 certification.

Submitting and payment

How to submit it
The completed form and any additional documents go to Sun Life by email, mail or fax, as listed on the form; incomplete or illegible forms are returned to the sender.
Who pays
The form states that the employee is responsible for any costs associated with its completion.

Good to know

  • Coverage continues only while the child stays disabled, unmarried and principally dependent on the parent for more than 50% of support, and the parent remains insured under the plan.
  • Section 4 asks yes or no whether the dependent is incapable of self-support because of a disability, plus the dependent's age when the disability occurred.
  • The physician is asked for as much detail as possible on the nature of the disability and a prognosis estimated in months or years.
  • Edition 5/23, two pages. The physician's signature certifies that the dependent is incapable of self-sustaining employment as a result of the details given.

Certification of Continued Coverage for a Disabled Dependent

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign Sun Life's disabled dependent certification?

Section 4 asks for the name and address of the dependent's primary care physician, and that physician signs Section 5. The signature certifies that the dependent is incapable of self-sustaining employment because of the disability described and authorizes release of information about the certification.

What makes a dependent eligible under GDIFM-11136?

Per the form's instructions, the child must be unmarried, incapable of self-sustaining employment because of mental or physical disability, and principally dependent on the parent for support, meaning more than 50%. Coverage lasts as long as these conditions hold and the parent remains insured.
