Prudential GL.2009.345: Visual Capacity Questionnaire

Prudential's group disability team sends this two-page questionnaire to the ophthalmologist or optometrist when a disability claim involves vision. The physician records best-corrected distance and near acuity, peripheral and central fields, hemianopsia, muscle function, depth and color perception, intraocular pressure and fundus findings, then the diagnosis with ICD codes and how often the patient can perform visual work tasks in an 8-hour day.

Who completes it

The treating eye care physician completes all three sections and signs with specialty. Prudential enters the claimant's name and claim number; the patient completes nothing on this form.

Submitting and payment

How to submit it
The completed questionnaire is faxed back to Prudential Disability Management Services, as printed on page 2, or mailed to the address in the header.
Who pays
The form sets no fee and does not say who pays for completing it.

Good to know

  • An ICD code is required for each diagnosis listed, primary, secondary and tertiary, and the physician marks whether the condition is permanent, stable, progressive, recurrent or expected to improve.
  • Workday tasks rated from never to constantly include walking on uneven surfaces, driving, climbing, operating handheld tools, eye-hand coordination, computer use, discriminating colors and reading instructions.
  • The form asks which accommodations would help, such as computer software, audio or voice-activated work processes, a non-industrial setting or extra time for tasks.
  • Edition 02/2016. The physician can also suggest additional tests that would help establish visual functional capacity.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who completes Prudential's Visual Capacity Questionnaire?

The treating ophthalmologist or optometrist completes it, since it asks for exam findings such as best-corrected acuity, visual fields, intraocular pressure and fundus exam. The physician signs the fraud certification and adds their specialty and printed name.

Does GL.2009.345 ask about color vision?

Yes. Item 5 records color perception as normal or limited, and whether a deficit is acquired, by eye. Discriminating colors and textures is also one of the workday tasks the physician rates from never to constantly.
