Prudential GL.2012.240: Capacity Questionnaire

Prudential sends this two-page capacity questionnaire to the treating physician during a group disability claim to pin down what the patient can do now. The physician states whether there is full-time or part-time transitional work capacity, estimates a full-time return date, grades 19 physical activities from never to constantly, and lists restrictions, limitations and accommodations that could increase work capacity.

Who completes it

The treating physician completes Sections 2 and 3 and signs the certification with specialty. Prudential fills in the claimant's name and claim number, which should appear at the top of each page.

Submitting and payment

How to submit it
The physician faxes the completed questionnaire to Prudential Disability Management Services; Prudential asks for a response within two weeks of receipt.
Who pays
The form sets no fee and does not say who pays for completing it.

Good to know

  • If no response arrives within two weeks, Prudential states it will base its opinion on the information already in the file.
  • Activities include standing, walking, sitting, climbing stairs and ladders, stooping, kneeling, reaching, handling, fingering and keyboarding by hand, and lifting up to 10, 20 and 50 pounds.
  • Restrictions are activities the patient should not do; limitations are activities the patient cannot do. The physician comments on the expected duration of each.
  • The certification asks whether the opinion rests on self-reported symptoms or objective findings, and whether vocational rehabilitation would help. Edition 9/2017.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long does a physician have to return Prudential GL.2012.240?

Prudential asks for a response within two weeks of receiving the questionnaire. Without a reply, it states that its opinion will be based on the information already available in the claim file, so a late response may mean the claim is decided without it.

What accommodations does Prudential's Capacity Questionnaire mention?

It gives examples of accommodative measures that may raise work capacity: adaptive devices such as voice recognition software, a foot-controlled mouse or lifting devices, working from home, scheduling flexibility and breaks during the workday, along with changes to the workplace, equipment or how work is done.
