Prudential GL.2022.201 Employer Review Capability Form

Prudential Disability Management Services uses this 2-page form to learn what a group disability claimant can do at work, so the employer can review return to work or modified duty. The health care provider completes it at every office visit, recording a full duty release date or a return date with restrictions. The form tells the provider not to disclose the patient's medical condition.

Who completes it

The employee's health care provider completes the whole form at each office visit: Section 2 on work capacity and restrictions, and Section 3 with provider details and a signature under the fraud notice. The claimant's name and claim number go at the top of each page.

Submitting and payment

How to submit it
Fax the completed form to Prudential Disability Management Services within 24 hours after each office visit, using the fax number printed on the form. Patients can also text a photo of a completed release to Prudential through its two-way texting service.
Who pays
The form names no fee, billing code or Prudential payment for completing it.

Good to know

  • Restrictions must be directly related to the disabling condition and detailed, for example a 25 lb weight limit, walking limited to 2 hours a day, or verbal-only instructions.
  • Expired restrictions and limitations count as a full duty release as of their end date, so give a Restrictions Needed Through date and an Estimated Full Duty Release Date.
  • Restrictions and limitations may not be based on Human Resources or employee relations issues; Prudential states this in the footnote to the work capacity section.
  • Section 2 offers return with no restrictions, return on a date with restrictions (full time, or part time with hours per day for a number of weeks), or permanent restrictions.

Frequently asked questions

Should I list the diagnosis on Prudential's Employer Review Capability Form?

No. The form states in capital letters that the patient's medical condition should not be disclosed when completing it, because it supports the employer's review of return to work or modified duty. Describe only the restrictions and limitations, how long they last, and the next office visit date.

How often does Prudential GL.2022.201 need to be completed?

The form is headed 'To Be Completed by the Employee's Health Care Provider at each Office Visit.' Prudential asks that it be faxed within 24 hours after each visit, so updated restrictions, the estimated full duty release date and the next visit date reach the claim promptly.

What happens when restrictions on the Prudential capability form expire?

The form's footnote says expired restrictions and limitations equate to a full duty release as of the end date. If the patient still needs restrictions, record a later Restrictions Needed Through date on the form completed at the next office visit.
