Prudential GL.2020.025: Supplemental APS

Prudential uses this supplemental statement to update an open group disability claim with the attending physician's current findings. It asks for clinical diagnoses with required ICD codes, pregnancy EDC and delivery date, the date significant loss of function occurred, claim type, other treating physicians, tests and procedures, treatment and prognosis, hospital stays, a return-to-work target and plan, medical and non-medical obstacles, and the job category matching the patient's abilities.

Who completes it

The attending physician completes Sections 2 to 4 and signs the fraud certification. Prudential identifies the claimant by name and claim number.

Submitting and payment

How to submit it
The physician returns it to Prudential Disability Management Services by fax or mail, as listed in the header.
Who pays
The form sets no fee and does not say who pays for completing it.

Good to know

  • The physician checks whether the disability is work related, an accident, a motor vehicle accident with the state, sickness or maternity.
  • Job categories are defined by weight: sedentary for negligible weight and mostly sitting, through light, medium and heavy, to very heavy for over 50 pounds frequently.
  • The form asks about non-medical factors that significantly affect functional abilities, such as interpersonal, financial or family issues, separately from medical obstacles.
  • The physician states whether the claimant is competent to endorse checks and direct the use of proceeds. Edition 04/2020; companion form GL.2003.251.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

What return-to-work details does GL.2020.025 ask for?

Section 3 asks for a return-to-work target date, full-time or part-time, with any limitations or functions lost, a description of the return-to-work plan, medical obstacles to returning, the nature of the impairment, and the job category that best describes the employee's functional abilities.
