Prudential GL.2021.170: Pulmonary Capacity Form

For a group disability claim based on a lung condition, Prudential asks the treating physician or pulmonologist to complete this three-page questionnaire. Who completes it

The treating physician completes Section 2 and signs the certification with specialty. The claimant's name and claim number go at the top of each page.

Submitting and payment

How to submit it
The physician faxes the completed questionnaire to Prudential Disability Management Services and is asked to respond within two weeks of receipt.
Who pays
The form sets no fee and does not say who pays for completing it.

Good to know

  • Attach the dates and results of 6-minute walk tests and pulmonary function tests; if results are unavailable, send the most recent office notes with exam findings.
  • Oxygen questions cover the flow rate and whether use is 24/7, nighttime or daytime, plus O2 saturation at rest and during activity or a walk test.
  • The form also asks whether palliative or hospice care is needed, whether improvement is expected and when, and whether there is cognitive impairment.
  • Without a reply within two weeks, Prudential bases its opinion on the file. Edition 11/2021.
