Prudential GL.2015.038: Psychiatric Work Readiness

Prudential uses this four-page assessment when a claimant's absence from work is attributed to a psychiatric condition. The physician works through screening questions on restrictions, acute risk, workplace conflict and change from baseline, then rates readiness in areas such as self-care, structured work, social skills, memory, concentration and adaptability, giving restrictions, a treatment plan for restoring readiness and an anticipated date for each.

Who completes it

The treating physician or psychiatrist completes the assessment and signs the certification on page 4. The claimant's name and claim number go on every page.

Submitting and payment

How to submit it
The physician faxes the signed assessment to Prudential Disability Management Services; several screening answers allow signing and faxing back early.
Who pays
The form sets no fee and does not say who pays for completing it.

Good to know

  • If the employee is actively suicidal with intent, actively homicidal or psychotic, the physician lists the symptoms, treatment plan and prognosis, then signs and faxes without completing the rest.
  • If the absence is mainly due to discomfort with a specific situation, coworker or supervisor, the physician gives recommendations and a readiness date and may stop there.
  • For any loss of function from baseline, Prudential asks for clinical evidence: observed behavior or performance, standardized test results, or other corroborating information.
  • The form defines a restriction as avoiding an activity due to risk, and a limitation as being unable to perform it. Edition 07/2015.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When can a physician stop early on Prudential's Psychiatric Work Readiness Assessment?

The form allows signing and faxing back early if there are no psychiatric work restrictions, if the employee is acutely at risk or psychotic (with symptoms and plan), if return is possible once a workplace conflict resolves, or if function has not changed from an adequate baseline.

What does Prudential mean by a treatment plan on GL.2015.038?

The instructions say a treatment plan should describe in detail the treatment methods that specifically target the psychiatric impairment. Each readiness area asks for that plan along with current restrictions and limitations and an anticipated date of work readiness.
