Prudential GL.2016.027: Kurtzke FSS for Life Claims

Prudential's Waiver of Premium Division sends this two-page form when a group life insured claims waiver of premium because of multiple sclerosis. Based on the latest neurological examination, the attending physician circles one Kurtzke grade for each functional system: pyramidal, cerebellar, brainstem, sensory, bowel and bladder, cerebral and visual, along with the years of diagnosis and first symptoms.

Who completes it

The attending physician or neurologist completes the scoring sections and signs the fraud certification. Prudential identifies the claimant by name and claim number.

Submitting and payment

How to submit it
The physician returns it to Prudential Group Insurance's Waiver of Premium Division by mail or fax, as listed in the form header.
Who pays
The form sets no fee and does not say who pays for completing it.

Good to know

  • Grades run from 0, normal, up to the most severe level, with a separate unknown option for each system.
  • In the cerebellar section, the physician records 1 in a small box when weakness of grade 3 or worse on pyramidal testing interferes with the exam.
  • Visual function grades depend on corrected acuity of the worse and better eye and field loss; a box records the presence of temporal pallor.
  • This form serves group life claims; Prudential's group disability claims use the separate Neurological Assessment of MS Diagnosis, GL.2012.239. Edition 4/2018.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
