Good to know

  • An ICD code is required for each diagnosis, with diagnosis date and expected duration or return-to-work date.
  • The physician states whether limitations are objectively or subjectively based, and what changes in the condition are needed before the patient can return to work.
  • A checklist covers difficulty with daily activities, chores, hygiene, memory, appointments, communication, concentration, fine motor tasks, holding a pen and keyboarding.
  • The certification has three outcomes: full-time return to the previous occupation, return with restrictions and accommodations, or unable to return to any occupation. Edition 10/2019.
