Sun Life GSTDFM-10784: PFML Care for Service Member

Employees taking state paid family and medical leave under a Sun Life private plan to care for an injured current service member need this certification from the service member's provider. It follows the FMLA military caregiver format but uses PFML family definitions, including siblings, grandparents and domestic partners. The provider documents service connection, severity and care needed.

Who completes it

The employer or employee completes Section 1 (dates requested and due); the employee completes Section 2 on the service member; the service member's health care provider completes Section 3, Parts A to C, and signs Section 5.

Submitting and payment

How to submit it
The completed form is mailed, emailed or faxed to Sun Life, as printed on its last page, with the claim number.
Who pays
The form sets no fee and names no payer for the provider's certification.

Good to know

  • Section 1 records the date the certification was requested and a return-by date, which must allow at least 15 calendar days.
  • Providers select DoD, VA, TRICARE network or non-network, or another 29 C.F.R. 825.125 provider, and may rely on a DoD recovery care coordinator's determinations.
  • The form prints fraud warnings for Connecticut, Massachusetts, Maine and Washington directly above the provider's signature line.
  • Revision 04/26 is current. Terms like 'lifetime' or 'unknown' may not be enough; estimate intermittent care over the next six months.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How is GSTDFM-10784 different from Sun Life's FMLA service member form?

GSTDFM-10784 is for state paid family and medical leave under Sun Life private plans, while GAMSFM-11125 is for federal FMLA. The PFML version lists broader family relationships, such as sibling, grandchild, grandparent and domestic partner, and adds an employer section with request and return dates.

Is use of GSTDFM-10784 mandatory?

The form itself says its use is optional; it asks the provider for the information needed for a complete and sufficient certification. An employer may require a timely and complete certification to support the leave, whatever form carries it.

What severity categories does the provider choose from?

Very Seriously Ill/Injured, Seriously Ill/Injured, Other Ill/Injured, or none of the above. The first two are internal DoD casualty assistance designations used by DoD providers; 'Other' means the condition may make the member medically unfit for duty.
