Sun Life GSTDFM-9859: PFML and STD Provider Statement

When an employee may qualify both for statutory leave and for the employer's short-term disability benefits, Sun Life gathers both in one provider statement: certification of a serious health condition, plus the disability details. Questions needed only for disability benefits are bold and marked with three asterisks. Sun Life uses it to decide the STD claim and may use it for other leave the employee qualifies for.

Who completes it

The patient completes Section 1; the health care provider completes Sections 2 to 7, covering diagnosis, treatment, restrictions, leave and return to work, and signs Section 9.

Submitting and payment

How to submit it
The completed form is emailed, mailed or faxed to Sun Life with the claim number; the employee must make sure it reaches Sun Life within 15 days of the absence request.
Who pays
The patient pays: the form states that the patient is responsible for any costs associated with completing it.

Good to know

  • Leaving the *** questions blank will not affect the statutory leave request, but it may delay or affect the short-term disability claim.
  • STD-only items include ICD-10 diagnosis codes, diagnostic test findings, restrictions and limitations, and return-to-work timing for part-time and full-time duties.
  • Terms such as 'lifetime', 'unknown', 'ongoing' or 'as needed' may not be enough to make a decision; give estimated dates and frequencies.
  • An appendix reprints the Department of Labor definitions of a serious health condition. The provider keeps a copy of the signed authorization on file.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who pays for completing Sun Life's GSTDFM-9859?

The patient. Section 1 states that the patient is responsible for any costs associated with completing the form, so any charge for the statement is the patient's responsibility rather than Sun Life's or the employer's.

Why are some questions marked *** on GSTDFM-9859?

Leave laws allow a narrower medical inquiry than disability insurance does. Questions marked *** are needed only for the short-term disability claim; skipping them will not affect the statutory leave request, but may delay or affect disability benefits.

What return-to-work information does the form ask for?

Section 7 asks when the patient can return to their part-time occupation and to their full-time occupation, either as a specific date or as a range from 1-2 weeks to 2 months or more, never, or other. Section 5 records restrictions and limitations.
