Sun Life GSTDFM-9857: PFML Care for Family Member

Sun Life uses GSTDFM-9857 when an employee covered by a Sun Life private or equivalent state paid family and medical leave plan needs leave to care for a family member with a serious health condition. The family member's provider confirms the condition category, the medically necessary care and whether leave is continuous, intermittent or a reduced schedule.

Who completes it

The employee completes Sections 1 and 2 and gives all pages to the family member's provider; that health care provider completes Section 3, Parts A and B, with license and NPI numbers, and signs Section 5.

Submitting and payment

How to submit it
The completed form is mailed, emailed or faxed to Sun Life, as printed on its last page, with the claim number; the employee must return it no later than 15 days after requesting leave.
Who pays
The form sets no fee and names no payer for the provider's certification.

Good to know

  • The form notes that state definitions of a serious health condition vary; the provider confirms one by checking a category, such as inpatient care, chronic condition or multiple treatments.
  • The form warns that some state or local laws may not allow disclosure of the diagnosis or course of treatment.
  • Part B covers continuous leave, intermittent episodes with frequency and duration, a reduced work schedule, planned treatments and referrals.
  • Fraud warnings for Colorado, Connecticut, Delaware, Massachusetts, Minnesota, Maine, Oregon and Washington are printed above the signature. Revision 04/26 is current.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long does the employee have to return Sun Life's GSTDFM-9857?

The employee instructions say the completed form must reach Sun Life no later than 15 days from the date the leave was requested. Missing information can delay processing, so the provider section should be complete when it goes back.

Does the family member need to authorize the provider?

Possibly. The instructions note that the family member may be required to complete an authorization form so the provider can release health information to the employee. The employee is asked to give the provider all pages of the form, including their completed Sections 1 and 2.

What intermittent leave details does GSTDFM-9857 ask for?

The provider gives start and end dates for the intermittent period and estimates how often episodes occur and how long each lasts, per day, week, month or year. A separate question covers a reduced work schedule in hours per day and days per week.
