Sun Life GAMSFM-11125: FMLA Current Service Member

When FMLA military caregiver leave for a current service member is administered by Sun Life, the service member's provider completes this form, which mirrors the federal WH-385. The provider confirms whether the injury or illness arose in the line of duty, classifies its severity and estimates the continuous, appointment and intermittent care needed.

Who completes it

The employee completes Section 1 (employee and service member information and care to be given); the service member's health care provider completes Section 2, Parts A to C, and signs Section 3.

Submitting and payment

How to submit it
The completed form goes to Sun Life by mail, fax or email, as printed on page 4, with the claim number; the employee must be given at least 15 calendar days to provide it.
Who pays
The form sets no fee and names no payer for the provider's certification.

Good to know

  • Providers select their type: DoD, VA, TRICARE network or non-network authorized private provider, or another FMLA health care provider under 29 C.F.R. 825.125.
  • Providers unable to make military-related determinations may rely on an authorized DoD representative, such as a DoD recovery care coordinator.
  • Severity is classified as Very Seriously Ill/Injured, Seriously Ill/Injured, Other Ill/Injured, or none of these, using DoD casualty assistance designations.
  • Terms like 'lifetime', 'unknown' or 'indeterminate' may not be enough; intermittent care is estimated as frequency and duration over the next six months.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

A DoD health care provider, a VA provider, a TRICARE network or non-network authorized private provider, or any health care provider as defined in 29 C.F.R. 825.125. The provider checks which type applies in Part A of Section 2.

What does 'line of duty' mean on GAMSFM-11125?

The provider indicates whether the injury or illness was incurred in the line of duty on active duty, existed before active duty and was aggravated by service in the line of duty, or neither. Military caregiver leave requires one of the first two.

Which Sun Life form covers a veteran instead of a current service member?

GAMSFM-11126 covers a covered veteran's serious injury or illness, mirroring the WH-385-V. GAMSFM-11125 is only for current members of the Regular Armed Forces, National Guard or Reserves, including those on the temporary disability retired list.
