Sun Life GSTDFM-11179: FLI Care for Family Member

Sun Life sells an insured Family Leave Insurance product that provides paid family leave where no state program applies, and this statement supports a caregiving claim under it. An employee caring for a family member with a serious health condition brings it to that family member's provider, who confirms the condition category, the care needed and the leave schedule.

Who completes it

The employee completes Sections 1 and 2 (employee and family member information, care to be given); the family member's health care provider completes Sections 3 to 5 and signs Section 6.

Submitting and payment

How to submit it
The completed form is emailed, mailed or faxed to Sun Life, as printed on its last page, with the claim number; Sun Life asks for it within 15 days of the leave request.
Who pays
The form sets no fee and names no payer for the provider's certification.

Good to know

  • Care must be medically necessary to qualify, and the provider describes the type of care, such as help with hygiene, nutrition, safety, transportation, physical care or psychological comfort.
  • Part B has two intermittent options: one for flare-ups of incapacity and one for office visits, appointments or treatment, each with its own frequency and duration.
  • The family member may need to sign an authorization before the provider can release health information to the employee.
  • The companion FLI Employee Claim Statement, GSTDFM-11178, has no clinician section. Revision 06/24 is current.

Frequently asked questions

What is Sun Life Family Leave Insurance?

It is an insured product Sun Life offers to employers to provide paid family leave where no state paid leave program applies. Claims for care of a family member under it use GSTDFM-11179, which the family member's health care provider completes.

Does GSTDFM-11179 need a diagnosis?

The provider selects a serious health condition category, such as inpatient care, incapacity plus treatment, pregnancy, chronic, permanent or long-term, or multiple treatments, and gives the onset date and expected duration. The form notes that some state or local laws may not allow disclosure of the diagnosis or course of treatment.

When is an NPI required on Sun Life's FLI form?

The provider section asks for license number and state, and lists the NPI number as needed when the provider practices outside Colorado. Phone, fax, email and type of practice or specialty are requested as well.
