CM-988 Black Lung Medical History and Examination

This is the core report of the DOL-sponsored complete pulmonary evaluation for a coal miner's black lung claim, which usually also includes a chest x-ray, ventilatory study and arterial blood gas test. The examining physician records work, family, medical and smoking history, symptoms and physical findings, then gives a reasoned opinion on respiratory diagnoses, their causes, and whether the miner is totally disabled for the last coal mine job.

Who completes it

The examining physician completes every section from A to F, types their name on the front page and signs section F. The miner helps with the employment history; when DOL supplies Form CM-911a, only blocks B.1.a and B.1.d are needed there.

Submitting and payment

How to submit it
File it through DCMWC's C.O.A.L. Mine Portal or by mail to DCMWC, together with the related x-ray, ventilatory and blood gas reports.
Who pays
DOL pays for the DOL-sponsored complete pulmonary evaluation, which only DOL-approved diagnostic providers perform.

Good to know

  • Answer every item, even if only with N/A. DOL asks for thoroughness to avoid follow-up questions; this version replaces the previous CM-988.
  • Block D.8.a must state whether the miner is totally disabled for the last coal mine job; a mild, moderate or severe label or an AMA class alone is not enough.
  • DOL's definition of pneumoconiosis includes chronic lung disease significantly related to or substantially aggravated by coal mine dust, such as COPD, emphysema and chronic bronchitis arising from that work.
  • If test results do not meet DOL's qualifying standards but you still diagnose total disability, explain why in D.8.b, and apportion each diagnosis's contribution in D.8.c.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What does DOL mean by total disability on CM-988?

Total disability is the miner's inability to perform their usual coal mine job, usually the most recent job held for at least one year, because of a respiratory or pulmonary condition. That is why block B.1.a asks for the job title, physical requirements and exertion level, and D.8 compares the impairment to that job.

Do I need to complete the employment history on CM-988?

If DOL provides Form CM-911a, the miner's own work history, review it with the miner and complete only blocks B.1.a (last coal mine job of at least one year) and B.1.d (protective equipment). If no CM-911a is available, take a brief work history from the miner and complete blocks B.1 and B.2.

Who can perform the CM-988 exam?

Only DOL-approved diagnostic providers perform the DOL-sponsored exam, and DCMWC publishes an approved provider list. The fillable form is on DCMWC's website, and the guide on its last pages explains what DOL expects in the employment, findings, diagnosis, etiology and disability blocks.
