CM-2907 Report of Ventilatory Study (Black Lung PFT)

This report records the pulmonary function (ventilatory) study done for a coal miner's black lung benefits claim. It captures FEV1, FVC and the FEV1/FVC ratio before and, if given, after a bronchodilator, along with predicted values, the miner's cooperation and understanding, and the equipment used. DCMWC uses the results, measured under the 20 CFR 718 Appendix B standards, to help decide eligibility.

Who completes it

The technician or physician administering the test completes the form and is named in item 13. A physician prints their name and signs the certification that the study was conducted and reported under DOL specifications.

Submitting and payment

How to submit it
Submit the form with its tracings through DCMWC's C.O.A.L. Mine Portal or by mail to DCMWC.
Who pays
The form sets no fee of its own; when the study is part of the DOL-sponsored complete pulmonary evaluation, DOL pays for that evaluation.

Good to know

  • Studies after January 19, 2001 must include flow-volume loops: three flow-volume tracings plus three volume-time spirograms, each labeled with the miner's name and DOL case ID.
  • If the spirometer cannot produce a flow-volume loop, say so in item 10, which is also where to note dyspnea, coughing, poor cooperation or an incomplete test.
  • MVV is optional. If measured, obtain it independently rather than calculating it from FEV1, with two tracings when values are within 10% of each other and three otherwise.
  • Note findings in block D5 of the CM-988 exam form when applicable. Standards are summarized on Form CM-2954a; current version revised April 2020.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What tracings must accompany CM-2907?

Three tracings of the flow-volume loop showing the entire maximum inspiration and forced expiration, and three volume-versus-time spirograms derived electronically from the loop. Each tracing must carry the miner's name and DOL case ID. If MVV is measured, attach two or three MVV tracings depending on how closely the values agree.

Do I need to give a bronchodilator for CM-2907?

The instructions treat it as optional, but if a bronchodilator is administered you must report the values both before and after it, corrected to BTPS, and explain the significance of the results in item 10. Predicted normal values go in column (d).

Can a technician administer the CM-2907 test?

Yes. Item 13 names the technician or physician who administered the test. A physician must still print their name and sign the certification that the studies were conducted and reported in compliance with Department of Labor specifications and that the results are accurate.
