CM-893 Certification of Medical Necessity (Black Lung)

Before DOL will reimburse oxygen, a hospital bed, a wheelchair or home nursing for a black lung beneficiary, the treating physician must request prior approval on this certification. It records the pulmonary condition, the equipment or service prescribed, the requested duration, and recent pulmonary function and arterial blood gas results. If granted, authorization lasts up to one year and is renewed through a recertification.

Who completes it

The treating physician completes the form and must sign and date item 13, certifying medical necessity for the covered pulmonary condition. A physician who is not the patient's regular or active treating doctor explains why in item 13b.

Submitting and payment

How to submit it
Upload the form and test results through DCMWC's C.O.A.L. Mine Portal under the category Medical, or mail them to DCMWC; uploading needs only the case ID, last name and date of birth, not registration.
Who pays
Approval authorizes DOL reimbursement for the equipment or services, and the supplier must be an actively enrolled OWCP provider to be paid directly.

Good to know

  • Home oxygen needs a pO2 of 60 mmHg or less on room air in a chronic state with matching pCO2 and pH; if the blood gas was drawn on oxygen, the standard is 80 mmHg.
  • A hospital bed needs FEV1 at or below 40% of predicted or chronic hypoxia with pO2 of 55 mmHg or less; attach PFT tracings with flow-volume loop and ABG strips.
  • All supporting tests must be dated within 2 months before the prescription. Requests for indefinite services or missing objective data are returned for more information.
  • Home care prescriptions need test data, why the patient is homebound, a visit schedule with frequency, and the provider type (PA, RN, LPN or RT). Wheelchairs are reviewed individually.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long does a CM-893 approval last?

If granted, the authorization covers a maximum of one year. Equipment and home nursing must be reviewed yearly or at the expiration date, using a new CM-893 marked as a recertification (renewal) in item 8a, with supporting test results dated within 2 months of the prescription.

Can I submit a CM-893 before the supplier is chosen?

Yes. DCMWC's CMN FAQ says the form can be submitted even if the supplier of the equipment or service is not yet known. To be paid directly, the supplier must be an actively enrolled OWCP provider; suppliers that have never enrolled can register through the OWCP Connect portal.

What if a CM-893 request is rejected?

DCMWC returns prescriptions for indefinite services or those without required objective test data. If a request is rejected because the patient's condition does not meet DOL's reimbursement standards, the physician may submit other medical evidence supporting the prescription, and all evidence will be considered.
