West Virginia DNR Order: The Orange DNR Card

West Virginia issues its out-of-hospital Do Not Resuscitate order on an orange card. EMS can only follow DNR orders written as medical orders, such as this card or the WV POST form; DNR requests in advance directives can be honored only by non-EMS providers. The card withholds CPR alone and does not limit other treatment.

Who completes it

An MD, DO, APRN or PA completes the card after discussion with the patient, or with the medical power of attorney representative or surrogate decision-maker when the patient is incapacitated.

Submitting and payment

How to submit it
The card stays with the patient. A copy of the whole card, top and bottom portions, should go to the WV e-Directive Registry; DNR cards are automatically opted in to the Registry under West Virginia law.
Who pays
The WV Center for End-of-Life Care supplies DNR cards and POST forms to health care provider offices free of charge.

Good to know

  • Patients cannot order cards themselves, cards cannot be mailed to individuals for completion, and a blank card should never be handed to a patient to fill out alone.
  • Order ahead: provider offices request cards through the Center's online request form, and orders take about two weeks to arrive.
  • The current card is the 2020 version, but older versions remain valid and can still be used.
  • Treating clinicians can view Registry copies 24/7 through the West Virginia Health Information Network portal.

Fill it with Vero

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

How do I get West Virginia DNR orange cards for my office?

Health care provider offices order them free from the WV Center for End-of-Life Care through the online request form on its Clinicians pages. Allow about two weeks for delivery. Patients cannot order cards directly; they get one from their clinician's office because the card is a medical order.

Should I use the WV DNR card or the WV POST form?

Both are medical orders EMS can follow. The DNR card covers only CPR if the heart stops or breathing stops. The Center describes the POST form as having more options than a DNR card, including treatment orders for when the patient has a pulse and is breathing, plus nutrition preferences.
