Vermont Prescribing Physician Follow-up Form

After writing a prescription under Vermont's Patient Choice at End of Life law, the prescribing physician reports back to the Department of Health on how the patient died. The one-page form asks for the patient's name and date of birth, the prescriber's name, and whether death came from the prescribed medication, the underlying illness, another cause such as terminal sedation or ceasing to eat or drink, or is unknown.

Who completes it

The prescribing physician completes and signs the form. The Department will accept it only when signed by the prescribing physician, whether or not they were present at the patient's time of death.

Submitting and payment

How to submit it
Mail it to the Vermont Department of Health, Vital Records, within 10 calendar days of the patient's death if the physician knows of it, or within 60 calendar days of writing the prescription.
Who pays
The form and the Department's compliance rule set no fee or billing code for this report.

Good to know

  • This form is tied to a rule: the Rule Governing Compliance with Patient Choice at End of Life (effective 6.1.16) requires prescribers to submit a Department-published follow-up form.
  • The rule's list of required content also includes the date the prescription was written, alongside the prescriber's name, the patient's legal name and date of birth, and cause of death.
  • If the cause of death is not known, the form asks you to mark Unknown rather than leave the question blank.
  • The Department links this report to the prescriber's earlier Physician Reporting Form to count deaths from ingestion versus other causes.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When is the Vermont Prescribing Physician Follow-up Form due?

Within 10 calendar days of the patient's death when the physician knows of it, or 60 calendar days from the date the prescription was written. The deadline comes from the Department of Health's Rule Governing Compliance with Patient Choice at End of Life and is printed at the top of the form.

Do I have to have been present at the death to sign the follow-up form?

No. The form says the Department accepts it only if the prescribing physician signs it, whether or not they were present at the patient's time of death. Report the cause as best you know it: Patient Choice (lethal medication), underlying illness, unknown, or other with a description.
