Vermont Patient Choice Physician Reporting Form

When a Vermont physician writes a prescription under the Patient Choice at End of Life law (18 V.S.A. Chapter 113), this two-page Department of Health form documents compliance. It records the patient's diagnosis and state of residence, the dates of the first and second oral requests and the written request, the physician's determinations and disclosures, the referral for a second opinion, and the steps taken when prescribing.

Who completes it

The prescribing (attending) physician completes and signs the whole form. The consulting physician records their own confirmation on the separate Consulting Physician Reporting Form.

Submitting and payment

How to submit it
Mail it to the Vermont Department of Health, Vital Records. The Department says its forms help meet the law's mandatory reporting requirements but that using these particular forms is not mandatory.
Who pays
The Department's forms and compliance rule set no fee or billing code for this report.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
