NJ MAID Attending Physician Follow Up Form (MAID-3)

New Jersey uses this two-page follow-up form to close out a medical aid in dying case after the patient's death. The Office of the Chief State Medical Examiner uses it when it receives an attending physician's compliance form or a report of a suspected aid-in-dying death, calling the attending physician to support an accurate, timely death certificate and statistical reporting.

Who completes it

Office of the Chief State Medical Examiner staff complete it during a phone follow-up with the attending physician, who supplies the cause of death, death certificate status and contact details; an OCSME investigator signs the authorization section.

Submitting and payment

How to submit it
OCSME staff complete it during the follow-up call, and the form lists filing with the Office of the Chief State Medical Examiner by mail or email. Missing compliance forms are requested within 30 days of the patient's death.

Good to know

  • Cause of death is recorded as the terminal illness with medical aid in dying, the terminal illness without it, or another cause that is specified.
  • If the death has not yet been filed in New Jersey's Electronic Death Registration System, OCSME staff guide the attending physician through the filing before continuing.
  • If filing status is unknown, OCSME notifies the Office of Vital Statistics and Registry about possible duplicate death certificates.
  • OCSME checks that the attending compliance form, the patient's written request, the consulting physician form and any mental health form have been received.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does the attending physician have to fill out the NJ MAID follow-up form?

Not alone. The form states it is intended for the Office of the Chief State Medical Examiner to seek additional information from the attending physician. Staff call the attending physician after receiving the compliance form or a report of a suspected aid-in-dying death, and the attending answers the questions on cause of death and filing.

What happens if the NJ MAID compliance forms were never filed?

During the follow-up, staff check whether the attending compliance form, a copy of the patient's request, the consulting physician form and any mental health professional form were submitted. If not, they request a complete report within 30 days of the patient's death.
