NJ MAID Attending Physician Compliance Form (MAID-7)

New Jersey's Medical Aid in Dying for the Terminally Ill Act requires the attending clinician to document each step of a request on form MAID-7. It records the eligibility determination, the patient's oral and written requests, mental status, the dispensing provider and the medication prescribed. Clinicians meet it after prescribing, and the Office of the Chief State Medical Examiner uses it to follow up for the death certificate.

Who completes it

The attending physician or advanced practice nurse who determined the person was a qualified terminally ill patient completes and signs the five-page form. The consulting physician and any mental health professional complete their own compliance forms, which are attached.

Submitting and payment

How to submit it
File it with the New Jersey Office of the Chief State Medical Examiner as soon as possible and no later than 30 days after the patient's death, by mail or by emailing digitally signed forms; an electronic submission process is forthcoming.

Good to know

  • The filing is complete only with three attachments: a copy of the patient's written request, the Consulting Physician Compliance Form and, if applicable, the Mental Health Professional Compliance Form.
  • Record the dates and times of the first oral request, the written request and the second oral request, and confirm the patient was offered a chance to rescind.
  • The attending may dispense directly if authorized and holding a current DEA registration; otherwise note how the prescription reached the pharmacist: in person, by mail or electronically.
  • After a death and filing, the Office of the Chief State Medical Examiner calls the attending with follow-up questions, recorded on the MAID-3 follow-up form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
