NJ MAID Mental Health Professional Compliance Form

When the attending or consulting physician refers a patient requesting medical aid in dying under New Jersey law for a determination of capability, the mental health professional records the result on form MAID-2. It states whether the patient is or is not capable and confirms that the attending physician was told in writing. It then joins the attending physician's compliance filing with the state.

Who completes it

The mental health professional who received the referral, such as a psychiatrist, psychologist or clinical social worker, completes and signs the one-page form, listing the referring physician's details and their own contact details.

Submitting and payment

How to submit it
Deliver the completed form to the attending physician, who appends it to the Attending Physician Compliance Form filed with the Office of the Chief State Medical Examiner no later than 30 days after the patient's death.

Good to know

  • The form is used only when the attending or consulting physician refers the patient for a capability determination; it is not required in every case.
  • Check boxes record whether the patient is capable or not capable, and whether the attending physician has been notified in writing.
  • The professional never files with the state directly; the form reaches the Office of the Chief State Medical Examiner as an attachment to MAID-7.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
