New Hampshire POLST Form: Yellow Medical Order

Printed on bright yellow paper, this portable medical order is for New Hampshire patients at risk of a life-threatening event because of a serious life-limiting condition or advanced frailty. Based on the national form, it records CPR, initial treatment, additional orders and medically assisted nutrition. Choosing No CPR makes it a DNR order under RSA 137-J:26 V(b), so no separate DNR is needed.

Who completes it

A physician, physician assistant or APRN completes the orders after a conversation with the patient or representative and signs Section F; the patient or representative signs Section E. A social worker, nurse or clergy member who helped is listed on page 2.

Submitting and payment

How to submit it
The original goes to the patient, who keeps it at home where emergency personnel can find it, and the provider keeps a copy in the record. Copies can go to the care team and the health care agent.

Good to know

  • The posted 2023 PDF is a sample; official bright-yellow forms are ordered from the Foundation for Healthy Communities through its order form page.
  • Attach the pink Portable DNR form if the patient has one. A living will or durable power of attorney for health care is not a DNR order.
  • The form does not expire and cannot be modified; to change orders, void it and complete a new POLST. Copied, faxed and electronic versions are valid.
  • Revision notes from July 2023 summarize what changed in the current version.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does a New Hampshire POLST count as a DNR order?

Yes, when No CPR is chosen. Section A states that this will constitute a DNR order and no separate DNR order will be required, citing RSA 137-J:26 V(b). Without that statement, a POLST is not a substitute for the pink Portable DNR, and a DNR bracelet or necklace requires a valid DNR order.

Who can sign a New Hampshire POLST?

A physician, physician assistant or advanced practice registered nurse. Forms are invalid if not reviewed and signed by a provider, and only providers authorized to sign POLST forms in New Hampshire may sign. Verbal orders are acceptable with a follow-up signature, and electronic signatures are valid.
