MA VSC109: School Bus and 7D Driver Medical Form

Every applicant for a Massachusetts school pupil transport (7D) or school bus driver certificate needs a medical examination on VSC109. The clinician answers 12 items covering diabetes and insulin use, implanted defibrillator, vision, hearing, oxygen saturation, epilepsy, limb loss, other physical or mental conditions, contagious disease and substance use, then certifies whether the applicant is medically qualified to drive school pupils.

Who completes it

A licensed physician, nurse practitioner or physician assistant, not a chiropractor, completes Sections B and C with an NPI and signature. The applicant signs the medical release in Section A.

Submitting and payment

How to submit it
Email the completed form to the RMV's school bus and 7D unit with the certificate application; incomplete forms are returned. The current version is VSC109 01/23.

Good to know

  • Insulin use or any hypoglycemic episode requires the separate VSC102 diabetes form from an endocrinologist, and an implanted defibrillator requires the VSC103 cardiovascular form.
  • Vision for 7D and school bus drivers is 20/40 in each eye with 120 degrees combined field; bioptic telescope users are not eligible.
  • Hearing passes with a forced whisper heard at five feet in the better ear, or average loss of no more than 40 decibels at 500, 1,000 and 2,000 Hz.
  • A current epilepsy diagnosis makes the applicant ineligible, and oxygen saturation must be above 88% at rest or with minimal exertion.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a chiropractor complete the Massachusetts 7D medical form?

No. VSC109 must be completed by a licensed physician, nurse practitioner or physician assistant, and the form states that chiropractors may not complete it. The clinician gives a Massachusetts NPI and signs the attestation that the information is true and complete.

Which follow-up forms can VSC109 trigger?

A yes to insulin use or any hypoglycemic episode requires VSC102, the Diabetes Medical Evaluation Form from a board-certified or board-eligible endocrinologist. A yes to an implanted cardiac defibrillator requires VSC103, the Cardiovascular Medical Evaluation Form completed by a medical doctor.

Does a daylight-only restriction affect 7D eligibility?

Yes. School bus and 7D drivers cannot hold a license with a G (daylight only) restriction. If corrective lenses are needed to reach 20/40 in each eye, a B restriction for corrective lenses goes on the license.
