MA MAB105: Intrastate CDL Medical Waiver Application

Commercial drivers who fail a federal medical standard can apply on MAB105 for a Massachusetts intrastate-only waiver under 540 CMR 14.05(1)(c). Waivers cover only five conditions: vision impairment, diabetes, cardiovascular disease, loss or impairment of a limb, and hearing impairment. The physician completes the section for the applicant's condition and certifies whether the driver can operate a commercial motor vehicle safely.

Who completes it

A medical doctor licensed in Massachusetts completes Section C, the relevant condition items 1 to 5 and the qualification statement, with NPI and Board of Registration in Medicine number. The applicant completes Section A and signs the medical release in Section B.

Submitting and payment

How to submit it
Mail the completed application to RMV Medical Affairs with a copy of a recent DOT medical exam (49 CFR 391.43) showing the driver qualifies only with an intrastate waiver, plus an employer letter confirming in-state commercial driving.

Good to know

  • Diabetes waivers need specific reasons the driver is not, or is no longer, at risk of hypoglycemic episodes and a recommended date for the RMV to re-evaluate.
  • Cardiovascular waivers exclude AHA functional Class III or IV disease, and a defibrillator placed for a sudden death event bars a waiver until six months without appropriate firing.
  • With air brakes, the driver must hear the compressor cut out and detect an air leak, so a profoundly deaf driver is limited to vehicles without air brakes.
  • Waivers never apply to school bus certificates, and holders get a W restriction (commercial driving within Massachusetts only, with a medical waiver) on the license.

Frequently asked questions

What conditions can a Massachusetts intrastate CDL waiver cover?

Only vision impairment (120 degrees combined field, 20/40 in either eye, color recognition), a diabetic condition, a cardiovascular condition without Class III or IV disease, loss or impairment of a limb, and hearing impairment, each only when the certifying physician states it will not interfere with safe commercial driving.

Can a physician request extra time on the CDL road test?

Yes, but only extra time. The request must come from a physician on letterhead describing the medical need for additional time for the pre-trip inspection and maneuvers. Applicants must still pass every part of the Commercial Driver Road Examination.

Does a MAB105 waiver allow interstate driving?

No. It covers commercial driving within Massachusetts only. Drivers who do not meet the federal vision or diabetes standards can seek an exemption for interstate driving through the federal government's own medical exemption program.
