MA RMV MAB102: Class D/M Vision Screening Certificate

Applicants for a Massachusetts Class D or M permit or license can bring MAB102 instead of taking the RMV vision screening. The ophthalmologist or optometrist records Snellen acuity, combined horizontal field, bioptic telescope details, diplopia and color vision, then certifies in Section C whether the applicant meets the state's minimum visual standards. The RMV accepts only the signed original within 12 months of the screening.

Who completes it

An ophthalmologist or Massachusetts-licensed optometrist completes Section B (screening data) and Section C (analysis, Massachusetts registration number and signature) and keeps a copy for 12 months. The applicant completes and signs Section A.

Submitting and payment

How to submit it
Give the signed original to the applicant to present at the RMV. Photocopies are refused, and the certificate is not accepted more than 12 months after the screening date.

Good to know

  • Standards printed on the form: 20/40 in either eye with at least 120 degrees combined horizontal field qualifies; 20/50 to 20/70 with 120 degrees qualifies with a daylight-only restriction.
  • Record acuity as plain Snellen values without plus or minus qualifiers or counting fingers. The suggested target size for the field test is 10mm.
  • Bioptic wearers need 20/40 through the telescope and 20/100 through the carrier and other lens, and the telescope must meet all five design criteria; licenses are daylight only.
  • Unresolved diplopia must be answered no, and red, green and amber discrimination yes. The doctor may recommend an RMV re-evaluation within the five-year license period.

Frequently asked questions

How long is a Massachusetts vision screening certificate valid?

The RMV does not accept MAB102 more than 12 months after the screening date recorded on it. After that, the applicant needs a new screening and a newly signed certificate. The RMV also requires the original with original writing; photocopies are refused.

Can drivers 75 and older use MAB102 at renewal?

Yes. Drivers aged 75 or older renewing a Class D or M license must either pass a vision test at an RMV Service Center or AAA office, or present a completed vision screening certificate that meets the RMV's standards during their appointment.

Is MAB102 the right form for a CDL applicant?

No. MAB102 covers Class D and M licenses. Applicants for a Class A, B or C permit or license use MAB103, the CDL Vision Screening Certificate, which applies the federal standard of 20/40 in each eye and 70 degrees of horizontal field in each eye.
