MA RMV MAB103: CDL Vision Screening Certificate

Applicants for a Massachusetts Class A, B or C commercial permit or license may use MAB103 to show they meet the federal vision standard in 49 CFR 391.41: 20/40 in each eye and in both eyes, at least 70 degrees horizontal field in each eye, and recognition of red, green and amber. The eye doctor records the screening data and certifies whether the applicant is visually qualified to drive commercial vehicles.

Who completes it

An ophthalmologist or Massachusetts-licensed optometrist completes Section B (screening data) and Section C (analysis, registration number and signature) and keeps a copy for one year. The applicant completes Section A and authorizes the doctor to discuss the form with the RMV.

Submitting and payment

How to submit it
Give the signed original to the applicant, who presents it at the RMV. Photocopies are refused, and the RMV will not accept the certificate more than twelve months after the screening date.

Good to know

  • Monocular drivers are not qualified under the federal standard printed on the form.
  • Massachusetts may waive the federal vision standard for intrastate driving with 120 degrees combined field, 20/40 in either eye and red, green and amber recognition; that waiver uses MAB105.
  • Record Snellen values without plus or minus qualifiers or counting fingers, note whether corrective lenses are needed, and list any conditions or treatment the driver must follow.
  • The federal government runs a separate vision exemption program for interstate commercial driving; a Massachusetts intrastate waiver covers driving within the state only.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What vision standard does MAB103 apply?

The federal standard in 49 CFR 391.41: distant acuity of at least 20/40 in each eye with or without correction, at least 20/40 with both eyes, at least 70 degrees of horizontal field in each eye, and the ability to recognize red, green and amber traffic signals.

Can a monocular driver get a Massachusetts CDL?

Not under the federal standard. Massachusetts may grant an intrastate-only medical waiver if the driver has at least 120 degrees combined horizontal field, 20/40 in either eye and normal color recognition, applied for on MAB105 with a DOT medical exam and an employer letter.

How long is the CDL vision certificate accepted?

Twelve months from the screening date. The RMV accepts only the original certificate with original writing, so the applicant needs a new screening and a new form if more than a year passes before the RMV visit.
