MA VSC102: School Bus/7D Driver Diabetes Evaluation

A Massachusetts school bus or 7D school pupil transport applicant who uses insulin or has had a serious hypoglycemic event must be cleared by an endocrinologist on VSC102 under 540 CMR 2.15. The endocrinologist reviews three years of history and answers questions on hypoglycemic unawareness, insulin stability, serious events, complications and diabetes education, then witnesses the applicant agreeing to strict glucose monitoring conditions.

Who completes it

Only a board-certified or board-eligible endocrinologist may complete Section B and sign the attestation in Section C, adding Board of Registration and NPI numbers. The applicant completes Section A and signs Section C in the endocrinologist's presence.

Submitting and payment

How to submit it
Submit it with a completed 7D School Pupil Transport Certificate Application. The examination is valid for six months, so a new form goes to the RMV or Department of Public Utilities every six months.

Good to know

  • A serious hypoglycemic event is one severe enough to interfere with ongoing activities or require another person's help; one in the past three years disqualifies the applicant.
  • Drivers test immediately before driving and every four hours on duty, and may drive only between 80 and 350; below 60, or 350 and above, requires endocrinologist clearance.
  • Logs from a memory-equipped meter go to the endocrinologist at each renewal and within 15 days of a serious hypoglycemic event, after which the certificate must be surrendered.
  • The applicant must give the endocrinologist a complete written history: insulin start date, hospitalizations, consultation notes, diabetes studies and any insulin reactions in the last three years.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete the VSC102 diabetes evaluation?

Only an endocrinologist who is board certified or board eligible. The form tells other physicians not to complete it. The endocrinologist must know the past three years of history through treatment, consultation with the treating physician, or review of a complete written history.

How often must VSC102 be renewed?

Every six months. The examination is valid for six months from the date performed, and the driver reviews their status with the endocrinologist and signs a new form acknowledging the special conditions of the 7D or school bus certificate each time.

Does every school bus driver with diabetes need VSC102?

No. Drivers with diabetes who do not use insulin and have never had a hypoglycemic episode do not need it. The general medical form, VSC109, asks about insulin and hypoglycemic episodes and directs drivers who answer yes to this endocrinologist evaluation.
