Vermont VS-017: Driver Eyesight Evaluation

Vermont DMV requires this examination when it needs an eye doctor's opinion on whether a person's eyesight allows them to get or keep a license. A Vermont-licensed optometrist or ophthalmologist answers every question on ocular coordination, peripheral fields, red, green and yellow color perception and acuity. The examiner then states whether vision is sufficient with lenses, without lenses, or not sufficient to drive.

Who completes it

The applicant's name, addresses, license number and birth details go at the top. A Vermont-licensed optometrist or ophthalmologist completes all examination questions and the summary, and signs with license number and date examined.

Submitting and payment

How to submit it
Return the completed form to Vermont DMV; it is void if DMV receives it more than six months after the examination date.
Who pays
The individual pays the examination fee, not the Department of Motor Vehicles.

Good to know

  • All questions must be answered in full, including whether the applicant has simultaneous binocular vision, alternating or fixed fixation, and diplopia.
  • Vermont requires at least 60 degrees of temporal field in each eye; if a field is less, the examiner must explain why.
  • The examiner states whether subnormal findings are progressive, how often progress reports should go to DMV, and whether to restrict the driver to daylight operation.
  • The Universal Medical Evaluation/Progress Report (VS-113) is the companion form when DMV needs a general medical report.

Frequently asked questions

How long is a Vermont VS-017 eyesight evaluation valid?

The form is void if Vermont DMV receives it more than six months after the date the optometrist or ophthalmologist examined the person. The examiner records the date examined next to the signature, so the completed form should be returned promptly after the visit.

Can an out-of-state eye doctor complete Vermont VS-017?

The form calls for an examination by a Vermont licensed optometrist or ophthalmologist, who records their license number and state of license. The examiner gives a decision, with due regard for public safety, on whether the applicant's eyesight is sufficient with or without lenses to operate a motor vehicle.
