Indiana BMV State Form 22106: Certificate of Vision

Indiana drivers who fail the vision screening at a BMV branch receive this Certificate of Vision (Eye Referral) to take to an eye doctor. The ophthalmologist or optometrist certifies the driver's visual acuity, and visual fields when needed, and the BMV's Driver Ability Department can then ask for more information, order a driving evaluation, add restrictions, set follow-up, or revoke privileges.

Who completes it

An ophthalmologist or optometrist completes the vision certification.

Submitting and payment

How to submit it
The applicant returns the completed certificate to a BMV branch.
Who pays
The BMV lists no fee for the certificate.

Good to know

  • Drivers with an annual or biennial vision restriction, known as J restrictions, must submit the certificate before the screening expiration date; the BMV mails a reminder 60 days before.
  • If acuity falls in category 6 or 7, visual fields are required: Humphrey 120 full field, Esterman monocular, HVF kinetic, Goldmann III 4E or an equivalent test reaching 60 degrees temporally.
  • Follow-up for vision cases may be set every 12 or 24 months.
