Indiana BMV State Form 13226: Bioptic Vision Cert

Indiana drivers who need a bioptic telescopic lens to drive start with this certificate. A low vision specialist evaluates the applicant against the BMV's bioptic standards, completes the certificate and refers the applicant to a BMV-approved bioptic rehabilitation program. After training, the rehabilitation center notifies the BMV and the applicant takes the BMV driving skills test.

Who completes it

An optometrist or ophthalmologist, acting as the low vision specialist, completes the vision certification.

Submitting and payment

How to submit it
Return it to the BMV. Bioptic drivers moving from another state submit it with current visual fields and their out-of-state driving record.
Who pays
The BMV lists no fee for the certificate.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
