Connecticut P-142SW: Seizure Medical Report (CDL)

Connecticut lets intrastate commercial drivers with epilepsy or another condition that may cause loss of consciousness apply for a waiver from federal rule 49 CFR 391.41(b)(8). This report gives DMV the medical picture: diagnosis and onset, last episodes of altered consciousness, seizure category, anti-seizure medications and their stability, findings and test results, and the examiner's opinion on safe commercial driving.

Who completes it

The applicant completes Part 1, acknowledging the eligibility criteria and signing a certification and release. A medical examiner (licensed physician, PA or APRN) completes Part 2 from a personal examination.

How to submit it
Submit all parts to DMV Driver Services by email or to the mailing address on the form, with the R-314 Intrastate Medical Waiver Application. Incomplete forms are returned to the applicant's mailing address.
