Good to know

  • Reporting is permitted, not required: Section 14-46 says any physician may report, and good-faith reporters are protected.
  • Impairment checkboxes include alcohol or substance abuse, dementia, cardiovascular, neurological, ophthalmologic, psychiatric, pulmonary or sleep apnea, traumatic brain injury and other conditions.
  • You certify a personal examination within the 90 days before the report, under penalty of false statement (CGS 14-110 and 53a-157b).
  • The full evaluation that follows uses the P-142M Medical Form; anyone else reporting an unsafe driver uses the sworn affidavit P-244.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Are Connecticut ER physicians required to report impaired drivers?

No. Under Section 14-46 of the Connecticut General Statutes, any physician may report a person with a chronic health problem that will significantly affect safe driving, or with uncontrolled recurrent unconsciousness. The P-142ER is the form for emergency room physicians who choose to report.

What does DMV do with a P-142ER report?

The report is for the information of the Commissioner in enforcing motor vehicle laws and is used solely to determine whether the person may operate a motor vehicle in Connecticut. Further medical information is collected on the P-142M Medical Form, which may be referred to the Medical Advisory Board.
