Virginia DMV MED 8: Missing or Impaired Limb Assessment

Virginia DMV requests this assessment when it has concerns about a non-commercial driver with a missing or impaired arm or leg. The clinician describes each impairment, its date and cause, any underlying condition that may progress, and whether reaction time, strength, range of motion or other functions are likely to impair driving. DMV weighs the report with test results and Medical Advisory Board guidelines.

Who completes it

The customer completes Customer Information and signs the Information Release Approval on page 1. A physician, physician assistant or nurse practitioner completes and signs page 3.

Submitting and payment

How to submit it
The provider faxes the completed MED 8 to DMV Medical Review Services or mails the original. If the driver received an Order of Suspension, DMV must have it before the effective date in the notice.
Who pays
Any charges for completing the form are the customer's responsibility.

Good to know

  • The clinician can recommend DMV knowledge or road retesting, an evaluation by a certified driver rehabilitation specialist, adaptive equipment, or a prosthetic or orthotic device for driving.
  • Page 3 asks separately whether the patient can safely operate a motor vehicle and a motorcycle, so answer both.
  • Drivers seeking a CDL disability waiver or hazardous materials variance must also submit the MED 30 application.
  • Virginia Code 46.2-208(B)(1) bars DMV from releasing the medical data to anyone other than a physician, physician assistant or nurse practitioner.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete Virginia MED 8?

Page 3 must be completed by a physician, physician assistant or nurse practitioner, who records their specialty, license number, expiration date and issuing state and signs. The driver brings the whole MED 8 and the DMV letter to the examination and authorizes the provider to send the report and clarify it with DMV.

What happens if a Virginia MED 8 is not returned on time?

If the driver received an Official Notice or Order of Suspension, DMV must receive the MED 8 before the effective date to avoid suspension. If the privilege is suspended, reinstatement also requires proof of legal presence if the driver has not already provided it.
