Virginia MED 41: Bioptic Telescopic Lens Certification

Virginia applicants who plan to drive with a bioptic telescope need this certification before DMV decides whether they may be tested. The eye doctor confirms the patient has had a prescription spectacle-mounted telescope for at least 60 days and has clinically shown the spotting, tracking, recall and lighting-adjustment skills listed on the form. The applicant separately certifies their own practice with the lens.

Who completes it

An ophthalmologist or optometrist completes and signs the Ophthalmologists/Optometrists Certification section, including the telescope's make, power and date received. The applicant completes Applicant Information and signs the Applicant Certification under penalty of perjury.

Submitting and payment

How to submit it
Mail the completed form to DMV Medical Review Services. The MED 44 information sheet asks applicants to send MED 40, MED 41 and the license application together, by mail or at a DMV customer service center.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
